About Putting Care at the Center
Putting Care at the Center is the annual conference of the Camden Coalition’s National Center for Complex Health and Social Needs. This year, the conference is being held in Portland, OR from October 15-17, and being co-hosted by CareOregon! The theme is “Complex Care: Meeting the Moment,” reflecting the necessity for coordinated, adaptive, and innovative responses to the evolving landscape of health and social needs. Lawyers, health care providers, community leaders, policymakers, social workers, people with lived experience, and anyone with vested interest in complex care solutions can enjoy the three-day program packed with opportunities to learn, collaborate, and network.

About the Camden Coalition
The Camden Coalition is a multidisciplinary, community-based nonprofit working to improve care for people with complex health and social needs in the city of Camden, across New Jersey, and around the country. They develop and test care management models and redesign systems in partnership with consumers, community members, health systems, community-based organizations, government agencies, payers, and more, with the goal of achieving person-centered, equitable care.
