Performance measurement and evaluation can significantly enhance the effectiveness and sustainability of interventions targeting social determinants of health (SDOH) at community health centers. On August 29, 2024, at 1 pm ET, the National Center for Medical-Legal Partnership (NCMLP), along with a panel of subject matter experts, introduced new performance measures and resources specifically designed for community health centers. These tools are designed to strengthen the sustainability and impact of medical-legal partnerships, positioning the approach as a key strategy for healthcare delivery system transformation and population health improvement.

Participants gained a comprehensive understanding of these new performance measures and learn how to effectively utilize them to improve the longevity and effectiveness of their MLP initiatives. Our experts discussed and illustrated key components of sustainability, including securing institutional buy-in, integrating MLPs into the healthcare delivery model, and mastering data collection and analysis.
