SDOH Academy Logo ColorThe Social Determinants of Health Academy is a six-month virtual training series designed to help health centers and primary care associations (PCAs) develop, implement and sustain SDOH interventions in their clinics and communities. Fifty-eight organizations were selected from an open application process to participate in the pilot year from September 2016 – February 2017. Led by eight organizations with expertise in a range of SDOH topics, program implementation, and evaluation, The SDOH Academy is the first national initiative to offer a coordinated curriculum on multiple community-based SDOH interventions. About The SDOH Academy

Where a person lives, plays, and works has a bigger effect on his health than genetics. The home he lives in, the streets he travels, and the education he receives help determine how healthy he will be. Health centers and primary care associations (PCAs) have long understood that healthy people and healthy communities require looking beyond a clinic’s doors, and they are increasingly adapting a range of interventions to tackle these social determinants of health (SDOH).

The Social Determinants of Health Academy is a six-month virtual training series pilot designed to help health centers and PCAs develop, implement and sustain SDOH interventions in their clinics and communities. Led by eight organizations with expertise in a range of SDOH topics, program implementation, and evaluation, The SDOH Academy is the first initiative to offer a coordinated curriculum on multiple SDOH interventions. Each of the organizations serving as faculty for the SDOH Academy has a National Cooperative Agreement with HRSA to provide training and technical assistance to support the development of SDOH interventions in the health center field.

The 50 health centers and PCAs selected for The SDOH Academy will participate in six virtual trainings each on a specific aspect of SDOH – from how to better screen patients for SDOH needs to how to build the health care and community team needed to address them. Through the varying expertise of the faculty organizations, various interventions and target populations will be addressed during each training. Participants in The SDOH Academy also learn from and alongside their peers, and network with a motivated group of SDOH-focused stakeholders.

The Trainings

*Participants must attend all six trainings and provide feedback through surveys after every training. The trainings will begin at 3 PM EST and are 75 mins in length.
